Please include the following details:

• Your company details (name, contact number, and email address)
• A summary of the works you wish to undertake
• The location of works
• A copy of your RAMS (Risk Assessment and Method Statement)
• A copy of your public liability insurance
• A permit will need to be completed and confirmation received from the center before works are completed.

If you are completing standard works(PPM or Reactive), please complete an Access form

If you are working at heights, please complete a Hazworks form

If you are completing HOT WORKS, please request a copy of the HOT WORKS form and Contractor Hazard information pack (CHIP)

Please note we aim to review your request and reply within 48 hours of receipt.
